I've discovered that it is re-booting the phone that causes the Outlook.com account to become invisible in Settings: Accounts & sync. While the account is invisible, I can't check its sync settings, but it behaves as if email is still syncing while the calendar is not. If there's no other fix, I'll have to try removing the Outlook.com app.

I got up to eight phone calendars, none of which I needed!
I've re-installed the Outlook.com app, cleared the data from Calendar and Calendar storage apps and added the Outlook.com account. The Outlook.com calendar is syncing fine and the Calendar initially had just one phone calendar in the Default account, but every time I look there is an extra phone calendar set to sync and visible by default.
Perhaps this is just a bug in the Calendar and the solution is simply not to open the My calendars menu unless really necessary.

Discussion #257871 never resolved the OP's problem and now I have it too. My Outlook.com calendar recently disappeared from the native Calendar app on my Xperia Pro (4.0.4) and my Outlook.com account became invisible in Settings: Accounts & sync, but was still present because Android told me that I could not add it as I was attempting to add a duplicate account.

I removed and re-added the Outlook.com account via the Outlook.com email app, but now every time I open the native Calendar app, I have an extra unwanted phone calendar in the default account. I currently have five six! It seems that the Calendar app isn't satisfied with my syncing and showing the Outlook.com calendar, nor is it satisfied with N phone calendars - it always wants N + 1!

I can collapse these unwanted phone calendars back to one by clearing the data from the Calendar and Calendar storage apps, but this results in my having to re-add my Outlook.com account and in any case it doesn't stop the phone calendars from breeding. I feel like the sorcerer's apprentice.

Is there a specific sequence of steps that will fix this?

The Outlook.com problem appears to have been caused by recent server instability.

It seems that my Calendar problems were not directly related to the Outlook.com problem, but due to my reconfiguring the Calendar over-zealously.

I recommend:

For the Phone calendar in the Default account, do not turn off:

the visibility since it doesn’t make any difference, or

the syncing otherwise the calendar will clone itself each time the My calendars menu is opened.

To ensure that new events are created by default in the desired calendar, simply select this calendar for the first event created and subsequent events should be created in this calendar by default.
