Camera does not focus correctly

The title basically tells it all. I have been experiencing this for quite some time. Sometimes the error can be "shut off" by restarting the camera app. At least for a while. But I also had the camera become unfocused on the middle of a movie.
Once the error occurred it had never repaired itself "by itself". I always had to restart the application.
I just installed the new system update to 5.1.1, but it didn't get better.

Anyone had this problem and had a solution?

Try clearing camera app cache:
- Go to Settings -> Apps -> All -> Camera -> Clear data and restart your phone.

Take a look at the camera lens, are there an scratches on it or is there any dirt or particles on the lens?

You could repair the phone with PCC but it might be a hardware issue

Well, it seems, that either I have to do this trick quite often, or maybe I was too optimistic about the solution.

One more detail : when I try, to take a picture, that requires macro, the camera continues to focus just fine (and the macro sign appears). If I go for something not macro, the sign disappears and the focus unfortunately disappears as well.
