remove the SIM card > restart your phone > do NOT put the SIM card back in, do you have access to your phone, if you do, back up everything and then repair it with SUS.
and yes the PUK is from the SIM card's network

install SUS on your PC > restart your PC > run SUS > turn off your phone > follow the steps, voila, done
